There are 5 ways in which clinicians encounter new patients at VUMC. On the outpatient side, there is a new patient office visit or an office consultation. On the inpatient side, you may see an inpatient admission, observation admission, or consultation.
In each of these encounter types, there are different coding levels based on your documentation. According to Medicare rules, what is the difference in required elements for the History and Physical Exam for the two highest levels of coding for any of these encounters?
