Discharge data can help reduce readmission and improve patient care and outcomes. A change will allow ED and IP Discharges in Storyboard for the past 365 days. Providers in Primary Care and Specialty Areas will see a new section in storyboard “ED, IP Discharges” that tracks Vanderbilt Health discharges and external discharges that are reported by the Tennessee Hospital Association (THA). Patients on the Vanderbilt Roster and TennCare Roster are both tracked by THA.
