Program ensures continuity of care for complex patients

Lou Ann Owens had lost count of how many times her husband Paul had been hospitalized by mid-2017, so when a team from the Vanderbilt Familiar Faces (VFF) program visited his room in August with an idea that might reduce his hospital stays and better manage his health, the couple immediately agreed to participate.

Team coordinates care for patients with many needs

When Eddie Dotson arrived at Vanderbilt University Medical Center last September, he weighed 627 pounds and truly embodied the label “morbidly obese.” He had so much fluid in his chest it was hard to breathe and his heart, liver and kidneys were failing.
