The Medication Reconciliation best practice advisory will be updated to display medications that need review or decision when there are three or fewer. This change, which affects inpatient providers, pharmacists and nurses, will take effect Nov. 27. The change should make it easier for inpatient clinicians to know when new medications have been added before admission medication list that need review and/or decision-making.
