Nurse-managed IV heparin protocol will transition from nurse orders to a MAR-based (machine-assisted restriction) protocol on Feb. 5.
Details:
- When shifting from provider-managed to nurse-managed IV heparin you will need to discontinue the protocol order and enter a new protocol order.
- The nurse-managed protocol orders will be able to start with a custom rate (i.e. for patients continuing from an OSH or for patients restarting heparin who had a previous therapeutic rate). This can be accomplished by answering the order question, ‘Start Infusion at a Custom Rate?’ as ‘Yes’ and entering the custom units/kg/hr starting rate:

- High dose WITH bolus (ex. DVT/PE, valve) – therapeutic goal PTT 60-110
- Low dose WITH bolus (ex. ACS, Afib) – therapeutic goal PTT 60-80
- Low dose NO bolus (ex. post-op, neuro/stroke, SICU, burn, trauma, ortho hand) – therapeutic goal PTT 60-80
- Additional options for high dose NO bolus PTT 60-110, continuation of therapy, and very low dose (non-therapeutic) PTT 40-60, will be created in the future.
