Attending physician or chief of staff co-signature required for inpatient, observation and hospitalized for extended recovery status orders

Beginning Sept. 4, in order to improve patient admission status order accuracy before discharge order placement, the attending physician or chief of staff co-signature is required on VUH patient status orders before a discharge order can be completed. (Note: Same day procedure patients are excluded from this requirement).

Details: 

  • Please ensure that your clinical documentation supports the status order placed. See the table below for status definitions and guidance on appropriate use.
  • Attending physicians must co-sign any new update status orders before discharge order placement.
  • Route the order to the appropriate attending physician to support a timely attending physician co-signature in Haiku.
  • If not routed to the appropriate attending, the covering clinician will have to sign the order from a workstation. 
  • When a discharge order cannot be completed because the attending physician has not co-signed the status order, and a suitable attending physician cannot be reached, contact the Chief of Staff on call to co-sign the status order.
Required Action(s): Document patient complexity that supports the admission status ordered.

Remember: Status order updates require attending physician co-signature. Contact the Chief of Staff on call if your attending physician is not available to co-sign a status order.
