To easily view and access a patient’s goals of care documentation and points of contacts, Advance Directives Navigator is changing to Goals of Care with several improvements.
To locate the Goals of Care Navigator, click the ‘Goals of Care’ link in Storyboard while in an encounter. Hover to discover details on the Storyboard ‘Health Care Decision Maker’
To anchor Goals of Care documentation into the navigator notes view, you must use the .ACP dotphrase. Customize note as desired but do not delete the title or any blue highlighted areas (this drives the anchoring automation). This does not apply to Children/Pediatrics.
To mark a patient’s point of contact active, (1) assign the appropriate role and then (2) click the active checkbox. Only one active Health Care Decision Maker is permitted at a time. Unclick the checkbox to inactivate.
Alternatively, (1) click the pencil to view more details and/or (2) mark active.
There are new roles for Patient Points Of Contact. Clinicians and staff can update these if needed. The three roles are: Patient Identified Decision Maker (no legal document; patient request) Health Care Agent (legal document) Surrogate (Provider Appointed) |
