Hospice in Place eStar and Operational workflow to change

The Hospice in Place (formerly called Hospice Scatter Bed) workflow will change enterprise-wide on Aug. 14.

To initiate a referral to Hospice, contact Case Management and enter a Hospice Certification Referral Note:

To place Hospice in Place orders, use the “Hospice in Place Navigator” to discharge/readmit the patient:

This tab is only for Hospice in Place, not discharges to an external inpatient hospice facility.

Vanderbilt Wilson County Hospital is not required to use this tab; at that facility, continue current process of fully discharging and readmitting with Case Management.

The Discharge and Hospice in Place Admit Orders are defaulted. Place or change other orders PRN.

Continue current Hospice in Place documentation (e.g. Discharge Summaries, Hospice Admission H&P, etc.)

Hospice in Place Go-Live Support will be available Monday through Friday, from Aug. 14 to Aug. 28 for all staff.
